ABSTRACT

Objective To explore the impact of blood-activatingand qi-nourishing therapy on the hypercoagulable state of rats after femur fracture. Methods One hundred and twenty SD rats were randomly divided into 4 groups, namely normal group, model group,low-molecular-weight heparin(LMWH) group, combination group (LMWH + Tongmai Decoction),30 rats in each group. The rat model of femur fracture was established. After successful modeling,LMWH group was given subcutaneous injection of LMWH 600 U/kg,and the combination group was given subcutaneous injection of LMWH 600 U/kg together with gastric gavage of Tongmai Decoction, the model group was given subcutaneous injection of the same volume of normal saline. The treatment lasted for 1-7 days after the surgery. The pathologic features of the left great saphenous vein were observed by HE staining method, and the relative volume, thickness and quantity of the blood vessels were also measured. The plasma D-dimer (D-D) and fibrinogen (FIB) levels were determined by biochemical analyzer, and the plasma levels of whole blood viscosity at low shear rate (WBV-lsr) and whole blood viscosity at high shear rate (WBV-hsr) were measured with hemodynamic detector. Results On day 7 after the modeling, less endothelium cells, agglomerative red cells, and large thrombi were found in the great saphenous vein tissue section of the model group under microscope.Compared with the normal group, the levels of D-D,FIB,WBV-lsr and WBV-hsr in the model group at various time points were increased, the difference being significant (P < 0.05).After 7-day treatment, the levels of D-D,FIB,WBV-lsr and WBV-hsr in LMWH group and combination group were lower than those of the odel group, and the decrease in the combination group was superior to LMWH group(P < 0.05).The blood vessel endothelium cells in the combination group were arranged neatly with same cellular width while without enlargement or swelling, the effect being superior to that of LMWH group. Conclusion Blood-activating and Qi-nourishing therapy can effectively relieve hypercoagulable state of rats after femur fracture.

ABSTRACT

Objective To explore the effect of traumatic brain injury on the fracture healing and its related mechanism by observing the expression of platelet-derived growth factors (PDGF) in serum and bone callus in rats with bone fracture and cerebral trauma. Methods One hundred and forty-four SD rats were randomized into 4 equal groups (n =36) which were subjected respectively to:no treatment (group N),traumatic brain injury (group TBI),bone fracture (group F) and bone fracture and cerebral trauma (group TBI + F).The animals were sacrificed at 3 days,1,2,3 and 4 weeks after modeling. In all the 4 groups,ELISA was used to detect the expression of PDGF in serum. In groups F and TBI + F,the callus growth was observed at the right tibial fracture site by X-ray,the callus growth and morphology were also observed by HE staining, the expression of PDGF in the callus tissue was measured by immunohistochemieal analysis, and the expression of PDGF mRNA in the callus tissue was measured by RT-PCR.Results X-ray showed that fracture healing was accelerated in group TBI + F compared with group F.The serum expression of PDGF in group TBI + F was significantly higher and the peak time was significantly earlier than in the other 3 groups (P < 0.05).H-E staining showed that osteoblastic activity at the fracture ends in group TBI + F was stronger than in group F.Inmunohistochemica[staining showed that the expression of PDGF in the local callus was significantly higher at 3 days and 1 week in group TBI + F and the peak time was significantly earlier than in group F (P < 0.05).RT-PCR showed that the expression of PDGF mRNA in the local callus was significantly higher at 3 days and 1 week in group TBI + F than in group F (P <0.05).Conclusions Traumatic brain injury can promote fracture healing in rats, which is probably related to increased expression of PDGF after cerebral trauma.

ABSTRACT

20 cases of artificial total hip joint rebuilding were selected, all of which were unilateral total hip joint rebuilding. Harris scores before and after the surgery and hip joint activity evaluation were carried out to the pa-tients. The average follow-up duration was 1 to 2 years. Up to the last follow-up,the activity of hip joint had been obviously improved compared with that before the operation. Through X-ray, the phenomena such as radiolucent zone around the prosthesis, prosthesis shift or osteolysis around the prosthesis haven't been seen. The short-term clinical effect of artificial total hip joint rebuilding was obvious for patients with hailure of first total hip joint re-placement caused by various reasons.

ABSTRACT

Objective To discuss the feasibility of positioning the acetabular center, fixing acetabular implant correctly and reconstructing hip rotation center according to Harris fossa and the remaining anatomical markers of acetabular notch in revision hip arthroplasty. Methods Twenty-eight patients underwent revision hip arthroplasty from April 2007 to June 2009.Based on Paprosky type,3 cases with type Ⅰ were treated with biological fixed acetabular component;8 cases with ⅡA and ⅡB were reconstructed with using of morselized bone grafting and large diameter cemented acetabular prosthesis;17 cases with type ⅡC,ⅢA and ⅢB were treated with using of morselized bone grafting and fixation of acetabular reinforcement ring.Among them,5 patients with massive bone loss in acetabular wall were reconstructed with the use of the structural and morselized bone grafting. The center of the original acetabulum was believed to be in the lunate cartilage surface which was closed to Harris fossa.During the operation, the center was located in the site which was 25-28 mm above in line with perpendicular bisector of acetabular notch connecting line. The acetabular center was the point of positioning acetabular prosthesis (Ⅰ type) or making new acetabulum by impaction bone grafting.Acetabular reinforcement ring (Ⅱ,Ⅲ type) was fixed in accordance with proper transverse angle and anteversion angle. The vertical distance from hip rotation center to teardrop connection and the horizontal distance from hip rotation center to teardrop were measured on preoperative and postoperative radiograph. And the outcomes of reconstruction of rotation center were evaluated. Results The vertical distance was changed from (14.22±3.39) mm preoperatively to (32.64±4.51) mm postoperatively. The difference was statistically significant (t=3.65,P< 0.05).The horizontal distance was changed from (25.13±3.46)mm preoperatively to (32.87±4.73) mm postoperatively. The difference was statistically significant (t=2.72,P<0.05).Conclusion Using residual Harris fossa and acetabular notch as the anatomical markers in revision hip arthroplasty, the restoration of the anatomical hip center has shown to be favorable.

ABSTRACT

Objective To investigate the diagnostic values of CT in xanthogranulomatous cholecystitis (XGC).Methods Retrospective analysis was performed in 4 patients with pathology confirmed XGC. Abdomen CT scans with and without contrast enhancement were performed in all patients. Results Only 1 case was correctly diagnosed before surgery. CT findings included gallbladder enlargement (3 patients), gallbladder wall thickening ( 4 patients), gallstone (1 patient), and common bile duct stone (1 patient). The maximum wall thickness was between 4 mm and 30 mm, with irregular low-density layer inside. Mucosal line was normal in 2 and pericholecystic infiltration in 1. After contrast administration, hypodense band sign was revealed in 2 and rim enhancement sign around the nodules was shown in 1 at arterial phase. Conclusion CT features of hypodense band sign and rim enhancement sign around the nodules in the arterial phase of contrast enhanced CT are strongly suggestive of XGC.

ABSTRACT

Objective: To investigate constitutional and phys ical stamina status in soldiers with different adaption periods in Tibetan areas . Methods: Two hundred and eithty-two male soldiers from a barr acks at an elevation of 3 600 m were divided into 2 groups: the veteran group ( n=143) in Tibet for 1.45±0.22 year, and the recruit group (n=139) in Ti bet for 0.33±0.00 years. The test items included body weight, cirumference, st a nding long jump, chin-up, 100 m dash and 3 000 m jogging, all were evaluated ac cording to the national military standard, which were Comprehensive Evaluation o f Health in Troops, and Examination and Evaluation of the Physical Stamina of So ldiers. Results: The physical stamina indexes of both the vetera ns and the recruits were up to the national military norm on the whole, ranking as moderate. The veteran group showed no significant difference in standing long jump and 100 m dash,(P<0.05), but obvious lower level in chin-up and 3 00 0 m jogging(P<0.01), compared with the national military norm. The recr ui t group showed significant lower level in chin-up, 100 m dash, 3 000 m jogging as compared with the national military norm(P<0.01), and also significant lower level in chin-up and 3 000 m jogging (P<0.05 or P<0.01) as compa red with the veteran group. Conclusion: The physical stamina of both the veterans and the recruits meet the basic national military requirements , ranking as moderate. The soldiers who have been in service for over 1 year hav e better explosive force, but they need more tolerance and aerobic exercises. Th e newly recruited need more exercise to raise tolerance to the hypoxic environme nt in plateau areas so as to shorten the adaption time to high altitude.
